From the SEELANGS listserv: 

On behalf of NAATPl (North American Association of Teachers of Polish), we are collecting information about programs currently offering Polish language instruction in North America. The ultimate goal is to have an aggregation of information about current programs, as well as the degree programs on offer to students. The information will be eventually placed on our web page (www.naatpl.org), primarily for reference, for language instructors and other interested stakeholders.
